The submission of graduate forms is very important. It is the primary way that the Graduate Coordinator's office can track degree progression. Questions about the forms may be directed toward the Graduate Coordinator Assistant, Bonnie Borgesi , or Enrolled Graduate Student Services.

Advisory Committee For M.S. and M.S.-NT. Students complete this form to designate their M.S. and M.S.-NT Advisory Committees. All committee members and the Graduate Coordinator must sign the form. The form is submitted to the Graduate School. Also, when changes to the Advisory Committee are needed, students need to complete and re-submit this form. The form is due by the end of the first semester since registration.
Application for Assistantship. Students need to complete the application to the best of their ability in order to be considered for an assistantship. Applications are turned into the Graduate Coordinator's office. Applications are due by February 1 each year.
Application for Graduation. Students need to visit this website and enter their graduation information, even if they're not 100% sure when they'll graduate. Be sure to email Bonnie Borgesi to let her know the application has been completed.
Application for Waiver of Graduate Out-of-State Tuition Out-of-state students complete and submit this form when all of their course work is completed and they only have the thesis or portfolio remaining. This form is signed by the student and the Graduate Coordinator and is submitted to the Graduate School.
Approval Form for Thesis, Defense & Final Exam Approval This form is used at the end of the Master's (thesis) degree pursuit. It is a multi-step form that is completed as the thesis is completed. This form is the final form that is completed and submitted to graduate. Students are responsible for filling out this form and obtaining the appropriate signatures at the appropriate time (see the Graduate Policies and Procedures Handbook for more details). The form is submitted to the Graduate School. The form is submitted to the Graduate School after all degree requirements have been completed.
Graduate Assistant Evaluation Form Faculty use this form to evaluate graduate assistants' job performance. It is to be completed at the end of each semester. The form is submitted to the Graduate Coordinator's office.
Graduate Student Annual Report Form Students use this form to update the Graduate Coordinator's office as to their activities from the past year. It also is designed to help graduate students get in the habit of keeping up with the things that they're doing. The form is submitted to the Graduate Coordinator's office.
Graduate Travel Funding Request College of Family and Consumer Sciences Students who will be presenting something at a conference can use this form to apply for financial support with travel expenses.
HACE Advisory Committee Action on Thesis Proposal Students use this departmental form during the thesis proposal. Students should complete this form (as best as possible) and take it to the thesis proposal meeting. Once all of the signatures are obtained, the form is submitted to the Graduate Coordinator's office.
HACE Non-Thesis Approval of Practicum Proposal Form Students use this departmental form to indicate the Advisory Committee's approval of the practicum proposal. The form is signed by the committee and submitted to the Graduate Coordinator's office.
HACE Non-Thesis Portfolio Presentation and Exam Form Students use this departmental form at the completion of their portfolio presentation. This form is the final form that is completed and submitted to graduate. Students are responsible for filling out this form and obtaining the appropriate signatures at the appropriate times (see the Graduate Polices and Procedures Handbook for more details). Recommended Change in Program of Study Students use this form when any change is needed in the Program of Study form. The form needs to be signed by the major professor and the Graduate Coordinator and is submitted to the Graduate School.
Request for Change in Degree Objective Students who have changed what degree they wish to pursue (M.S. to M.S.-NT or visa versa) use this form to indicate that change to the Graduate School. It is signed by the major professor and the Graduate Coordinator and is submitted to the Graduate School.
Request for Leave of Absence From Graduate Enrollment Students use this form when continuous graduate enrollment is not possible. The Application for a Leave of absence must be received by the Graduate School Office of Enrollment Services
on or before the last day of classes for the semester for which it is requested. A leave of absence will not
be granted retroactively after the end of a semester. The form must be signed by the major professor and the Graduate Coordinator.

Advisory Committee For Doctoral Students Students complete this form to designate their doctoral Advisory Committees. All committee members and the Graduate Coordinator must sign the form. Application for Admission to Candidacy for Doctoral Degrees This form is completed by students and taken to the oral comprehensive exam. If the oral comprehensive exam is passed, the Application for Admission to Candidacy form is signed by the student, major professor, and Graduate Coordinator. Three copies (original plus 2 copies) are attached to the oral examination form and submitted to the Graduate School within 2 weeks of the oral exam's date.

Doctoral Commencement Information This online form is used to notify the Graduate School that the doctoral student will be participating in commencement. The information gathered from this form is used to create the commencement programs. If students do not fill out this form in a timely manner (check the Graduate School's deadlines), their names will not be included in the commencement program.

This form must be signed and all remedial work completed before the oral comprehensive exam announcement can be sent to the Graduate School. The form is signed by the Advisory Committee and submitted to the Graduate Coordinator's office.
HACE Ph.D. Teaching Request Form Students complete and submit this form to show that their request for teaching experience has been completed. The form is signed by the taught course's supervising faculty member, the Advisory Committee, and the Department Head. The form is submitted to the Graduate Coordinator's office.
Notification of Intent to Take Ph.D. Written Comprehensive Exam Students complete and submit this form to indicate their plans for taking the written comprehensive exam. The form is signed by the student, major professor and Graduate Coordinator. The form is submitted to the Graduate Coordinator's office.
Preliminary Program of Study for Ph.D. Students in HACE (PPOS) Doctoral students are required to submit a Preliminary Program of Study to the Graduate Coordinator's office. The PPOS is a detailed "road map" that shows students, Advisory Committees, and the Graduate Coordinator the plan for degree completion. The PPOS goes into much more detail than a final Program of Study and illustrates departmental requirements for degree completion. The form is signed by the Advisory Committee and the Graduate Coordinator. The form is submitted to the Graduate Coordinator's office.
